How Do I register?

To register on the Regent Street Dental Care Plan:

Please call in at the practice where an agreement will be completed. This will require your signature and bank account details for the Direct Debit Mandate. Direct Debits are collected from your account on the 8th of every month. Any patient enrolling before the 27th of each month will benefit from cover from the 8th of the following month.

If you wish to register as a private patient:

Please call the practice and book an appointment. We will ask you to attend the appointment 10 minutes before you are due to attend so that we can fill out a registration form and an up to date medical history can be taken.
